To be considered as a provider of services or supplies we ask all interested parties to file a vendor application and IRS W-9 form which we enter into our consultant roster database.  Within that vendor application are several dozen selections for the types of services or supplies you wish us to consider your company for.  I encourage you to complete and return both forms at your earliest convenience.

Please also note there are TWO different vendor applications.  One for all interested vendors and one specifically for Public Works Projects.
