TCCAP CHWs are responsible for bridging conversations with patients and removing barriers that prevent individuals from accessing health and social services. CHWs meet their patients through in-person or online outreach and create a care plan with the patient to practice wellness activities that support their resiliency. The position may be part-time at community provider offices, housing navigation centers, and home offices. TCCAP’s Program Coordinator role will oversee and implement health and wellness programs, including creating and maintaining connections between health and social services. The position requires individuals to work closely with community providers to respond to clients who engage in services addressing barriers to community health and wellness. The role will coordinate closely with the Community Action Department (CAD) Program Coordinator.
