The Glenn County Child Health and Disability Prevention (CHDP) Program is a preventive health program that is administered by Children's Medical Services (CMS) under Federal and State mandate. It provides free CHDP Health Exams for low to moderate income infants, children, teens, and youth.
These no cost complete health assessments promote early detection and disease prevention for those with obvious health problems and those who appear well. Children may receive well-child exams according to the CHDP Periodicity Schedule; from birth through 20 years if they have Medi-Cal and from birth through 18 years if they have no Medi-Cal but have an eligible income. The Health Exam is provided by CHDP approved private medical doctors, clinics, medical groups, comprehensive health centers, and hospital clinics.

CHDP Health Exams

Qualifying for CHDP Services
Child has no-cost Medi-Cal
Your child may get free exams up to his or her 21st birthday. Medi-Cal will pay for follow-up and treatment for health problems found during the exam.
Child has Medi-Cal with a Share-of-Cost
Your child may get free exams up to his or her 19th birthday if:
- You have not met the share-of-cost (you have no coverage through Medi-Cal for the CHDP health exam on the date of service) and
- Your family is income-eligible
Family’s income is low to moderate
Your child may get free exams up to his or her 19th birthday if:
- Your family has an eligible income (equal to or less than 200 percent of the federal poverty level)
- Your child does not have health insurance or their insurance only pays for sick care and does not pay for well child exams
- Your child does not have coverage through Medi-Cal
Children in Foster Care are eligible
Children who attend Head Start/State Preschool (3-5 years of age) are eligible
Obtaining a CHDP Exam
- Call Public Health's CHDP office for the names of CHDP providers nearest you or call your physician and ask if they provide CHDP exams
- Make the CHDP exam appointment
- Arrange travel for the appointment
- Families with Medi-Cal must bring their Medi-Cal cards, families that have no insurance must complete and sign a form declaring their income at the time of the visit
- Families of children attending a Head Start or State Preschool program should let the school and provider know when going in for a school entry health exam
School Entry Health Exams
- All children entering kindergarten or first grade should have a complete health exam and up-to-date immunizations. Approximately one in ten children starting school has an unknown health problem such as asthma, hearing loss, or vision loss. A comprehensive physical exam increases the opportunity to detect health problems early so that the conditions may be treated and long-term complications may be prevented.
- When you take your child in for a school entry health exam (up to 18 months before first-grade entry), ask the doctor to fill out a "Report of Health Examination for School Entry" form. Then take the completed report to your child's school nurse or school secretary at enrollment.
- Children from low and moderate income families may get their exam and immunizations at no charge through our CHDP providers in Glenn County. If you child needs immunizations, he or she will get them during the health exam.

Medi-Cal
Medi-Cal for Children
There are three types of Medi-Cal for children in Glenn County:
- No Cost (Full-Scope) Medi-Cal
- Restricted Benefits (Limited Scope) Medi-Cal
- Share-of-Cost Medi-Cal
All children with no-cost Medi-Cal are eligible for all CHDP (well-child) exams and follow-up care.
Children with restricted benefits or share-of-cost Medi-Cal may qualify for free CHDP exams with eligible income.
CHDP Nurse and Case Manager
Healthy and productive children and families are the foundation of our community.
The role of the CHDP Nurse focuses on the prevention of illness, the promotion of health, and support of the health promotion system. After the initial health assessment, children and youth with suspected health problems are assisted to care by the CHDP nurse.
The Case Manager focuses on the CHDP clients and providers through a process of assessing, planning, organizing, and monitoring services and referrals that meet the multiple needs of clients while supporting the effective use of health services.
Both the Nurse and Case Manager in the CHDP Program seek to improve the health and well-being of our community by advocating for children when providing case management services, health education (including resource referrals) and by coordinating measures of prevention.
The mission of the Nurse and Case Manager in the CHDP Program is to enhance the health and well-being of children in our community. This is accomplished by assuring that Medi-Cal eligible and low-income children acquire and effectively utilize available health care resources. In doing so, we foster a work environment that encourages creativity, diversity, teamwork, accountability, continuous learning, celebration of achievement and the highest ethical standards.
